Pain On Top Of Foot: Foot Pain Causes

Top of foot pain is a common painful injury that occurs during walking and running. This is characterized as foot pain that occurs during walking and running.  Extensor tendonitis is common due to tight-fitting running shoes that increased stress on the top of the foot and toes during weight-bearing.

Causes and Conditions

Extensor Tendonitis

Extensor Tendonitis causes top-of-foot pain when the extensor tendons that flex the feet and lift the toes become inflamed, tight, and injured during walking and running. Tight-fitting shoes paired with repetitive stress can cause injury to the tendons of the foot which eventually causes tendonitis (acute) or if chronic tendon degeneration (tendinosis). This top of foot pain is characterized by increased painful motion as activity increases.

Sinus Tarsi Syndrome

Top of foot pain caused by the ligaments and joint capsule of the channel of the sub-talar joint. Often associated with ankle injuries and can lead to instability.

Tight-Fitting Shoes

Narrow shoes that shoelaces are pulled too tight can cause compression on the tendon that’s lift the toes and ankle. The narrow toe box limits the ability for the toes to extend, and the tight laces create additional friction over the outer top foot. This overtime can cause a callus to form cause top of foot pain. An easy remedy is to buy wide toe box shoes and incorporate barefoot training into daily activity.

Peroneal Nerve Entrapment

The peroneal nerve can become entrapped in the lower third of the leg and cause the top of foot pain during running. This is common after an inversion ankle sprain due to the sudden excessive stretch on the nerve and can cause foot and ankle pain long after the ankle sprain has healed.

An easy way to assess for entrapment is to skin roll the lower third of the leg. It’s considered positive when it’s painful to lift the skin and creates top of foot pain. This skin rolling could also serve as a treatment for foot pain.
